Effective May 15,2007 we will collect your copay before you are seen by the Doctor. If you do not bring your copay, you will need to reschedule your appointment.
DEDUCTIBLES may be collected prior to surgery as indicated by your insurance company. For your convenience, in addition to cash or personal check, we also accept VISA, MasterCard, and Discover cards.
Please be aware that checks returned for insufficient funds will result in a $25.00 fee added to your account.
